London-wide NHS video campaigns for the Universal Care Plan

Universal Care Plan (UCP) is an NHS service based in South West London, that allows patients to generate and share a care plan digitally with all healthcare service providers across the NHS. We created a series of short videos to promote the Pan London NHS Digital Personalised Care Planning Programme - Universal Care Plan. Our next campaign was co-created with Londoners and will be displayed in GP’s surgeries soon.
