NHS App Walkthrough Explainer for the Universal Care Plan (UCP)

nooh Studio worked with the NHS Universal Care Plan (UCP) team to create an animated walkthrough video helping patients access and update their UCP through the NHS App. The Universal Care Plan allows people to share information about their health, care preferences and support needs with professionals involved in their care. To support patient engagement, we transformed a complex digital process into a clear, step-by-step guide, combining animation, screen recordings and accessible language. The final video helps patients understand how to access their care plan, update information and navigate the different sections within the NHS App, encouraging greater confidence and participation in their own care.
